a. HEALTHGO MARKET INC.: Violation of Rule 10.2 Manager on duty,
qualifications.
3. NEW LIQUOR LICENSE:
POINT HB LLC DBA LEI HANALEI: Application No. 2021-014 filed on
December 7, 2020 by POINT HB LLC dba LEI HANALEI for a New Class and
Kind (Retail Dealer General) at the former Harvest Market, located in Hanalei town.
The address is 5-5161 Building F, Kuhio Highway, Hanalei, Kauai, Hawaii.

4. CONVERSION FROM A CORPORATION TO A LIMITED LIABILITY
COMPANY
ALOHA PETROLEUM, LTD. (“ALOHA) Pursuant to HRS 281-41(k) Aloha
Petroleum, Ltd. requests to convert from a corporation to a limited liability
company. All required paperwork has been filed to include filing with the Hawaii
Department of Commerce & Consumer Affairs and will be effective on January 1,
2021

session shall be held pursuant to H.R.S. §92-4 and shall be limited to those items described in
H.R.S. §92-5(a).

Prior to the Day of the Meeting: Persons wishing to testify are requested to register their name,
phone number, and agenda item via email at mmarrotte@kauai.gov or by calling 808 241-4918.
On the Day of the Meeting: Persons who have not registered to testify by the time the
Commission meeting begins will be given an opportunity to speak on an item following oral
testimonies of registered speakers. The length of time allocated to person(s) wishing to present
verbal testimony may be limited at the discretion of the chairperson or presiding member.
