1.THE SCOPE OF WORK FOR THIS PROJECT INCLUDES SPALL, DELAMINATION,EXPOSED REINFORCING, AND CRACK REPAIRS IN THE DECK, GIRDERS, FLOORBEAMS, ABUTMENTS, PIER WALLS, AND RAILINGS. THE EXISTING AC AND BASECOURSE WILL BE REMOVED, AND A NEW CONCRETE DECK WILL BE PLACED ONTOP OF THE ORIGINAL DECK ALONG WITH AN ASPHALT OVERLAY. IN ADDITION,THE ELEVATED WALKWAY ON THE MAKAI SIDE OF THE BRIDGE WILL BEREMOVED AND REPLACED WITH NEW PREFORMED CONCRETE SLABS FOR THEWALKWAY SURFACE AND SIDES, NEW SUPPORT BRACKETS, AND NEW METALRAILINGS WITH ADDITIONAL HORIZONTAL MEMBERS TO MEET EXISTINGBUILDING CODES DUE TO THE LARGE OPENINGS IN THE EXISTING RAILINGDESIGN. THE BRIDGE SHALL BE SIMILAR TO THE ORIGINAL 1927 DESIGNMATCHING THE EXISTING COLOR.2. ALL CONSTRUCTION WORK IS TO BE CONSTRUCTED IN ACCORDANCE WITH THEPUBLICATIONS "HAWAI'I STANDARD SPECIFICATIONS FOR ROAD AND BRIDGECONSTRUCTION, 2005" AND ITS AMENDMENTS AND THE "STANDARD DETAILS FORPUBLIC WORKS, CITY AND COUNTY OF HONOLULU, AND THE COUNTIES OF KAUA'I,MAUI, AND HAWAI'I." THE STANDARD DETAILS ARE AVAILABLE AT THE COUNTYOF KAUA'I CLERK'S OFFICE.3. CONSTRUCTION PLANS ARE VALID FOR A PERIOD OF ONE YEAR FROM THE DATEOF APPROVAL. IF CONSTRUCTION DOES NOT COMMENCE WITHIN THE ONE-YEARTIME FROM THE DATE OF APPROVAL, THE CONSTRUCTION PLANS SHALL BERESUBMITTED TO ALL REVIEWING AND APPROVING AGENCIES FOR REVIEW,APPROVAL, AND RECERTIFICATION OF THE PLANS.4. ALL GRADING, GRUBBING AND STOCKPILING WORK SHALL BE PERFORMED INACCORDANCE WITH COUNTY OF KAUA'I ORDINANCE NO. 808.5. AFTER EACH RAINFALL EVENT, THE CONTRACTOR SHALL REMOVE ALL SILT ANDDEBRIS RESULTING FROM HIS WORK AND DEPOSITED IN DRAINAGE FACILITIES,ROADWAYS, AND OTHER AREAS. THE COST INCURRED FOR ANY NECESSARY REMEDIAL ACTION BY THE COUNTY ENGINEER SHALL BE PAYABLE BY THE CONTRACTOR.6. DURING CLEANING OPERATIONS, THE CONTRACTOR SHALL SUPPLY A WATERTRUCK FOR DUST CONTROL PURPOSES UNTIL THE VEGETATION HADRE-ESTABLISHED ITSELF. EXCESS WATER, INCLUDING SILT AND DIRT, SHALLNOT BE ALLOWED TO RUN-OFF THE PROPERTY.7. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL OVERTIME AND/OR NIGHTWORK PAYMENTS FOR COUNTY'S STAFF AND INSPECTION PERSONNEL INCLUDINGCONSULTANTS, WHEN THE CONTRACT REQUIRES OVERTIME OR NIGHT WORK TOBE PERFORMED, OR DIRECTS THE CONTRACTOR TO WORK ADDITIONAL SHIFTSOR OVERTIME FOR COUNTY'S CONVENIENCE.8. BEST MANAGEMENT PRACTICES (BMP'S) SHALL BE EMPLOYED AT ALL TIMES TOTHE MAXIMUM EXTENT PRACTICABLE TO PREVENT DAMAGE BY SEDIMENTATION,EROSION OR DUST TO STREAMS, WATERCOURSES, NATURAL AREAS AND THEPROPERTY OF OTHERS.9. IF SYSTEM CONDITIONS REQUIRE NON-EMERGENCY NIGHT TIME WORK DURINGTHE AUTUMN SEABIRD FALL SEASON (SEPTEMBER 15 THROUGH DECEMBER 15),USE OF LIGHTING SHALL BE RESTRICTED BETWEEN 9:00 P.M. TO 4:30 A.M. IFLIGHTING OF THE WORK AREA IS REQUIRED IN SUCH SITUATION, ALL LIGHTSSHALL BE SHIELDED (MINIMUM LIGHT SPILL TOWARDS THE SKY) AND DIRECTEDDOWNWARDS TO THE MAXIMUM EXTENT PRACTICABLE. MINIMUM REQUIREMENTSFOR LIGHTING BY HIOSH AND OSHA SHALL BE PROVIDED AND ASSURED BY THECONTRACTOR. THE CONTRACTOR SHALL TRAIN ALL EMPLOYEES WORKING ATNIGHT (RECORDS RETAINED BY THE CONTRACTOR) ON HOW TO HANDLE ANYRETRIEVED DOWNED BIRDS AND SHALL HAVE APPROPRIATE EQUIPMENT ASAPPROVED BY SAVE OUR SHEARWATERS (SOS) ON SITE TO HOLD ANDTRANSPORT ANY RETRIEVED BIRDS TO A SOS FACILITY. THIS REQUIREMENTDOES NOT ALLOW LIGHTING AS MAY BE RESTRICTED BY OTHERGOVERNMENT AGENCIES.10. PRIOR TO STARTING ANY EXCAVATION ACTIVITIES, THE CONTRACTOR SHALLCONTACT THE HAWAI'I ONE CALL CENTER AT 1-866-423-7287.11. THE DEPARTMENT OF PUBLIC WORKS SHALL BE PROVIDED TWENTY (20)WORKING DAYS FOR ALL SUBMITTAL REVIEWS FROM THE TIME OF SUBMISSIONBY THE CONTRACTOR.12. PRIOR TO CONSTRUCTION THE CONTRACTOR SHALL CONTACT THE VARIOUSUTILITY AGENCIES FOR LOCATION OF EXISTING UTILITIES WITHIN THE PROJECTLIMITS. THE CONTRACTOR SHALL LOCATE AND PROTECT ALL EXISTING UTILITIESWHETHER OR NOT SHOWN ON THE PLANS. ANY COST INCURRED BY DAMAGES TOEXISTING UTILITIES WILL BE BORNE BY THE CONTRACTOR.NOTES FOR GENERAL CONSTRUCTIONNOTES FOR CONSTRUCTION WITHIN COUNTY RIGHT-OF-WAY13. ALL EXISTING UTILITIES, WHETHER OR NOT SHOWN ON THE PLANS, SHALL BEPROTECTED AT ALL TIMES BY THE CONTRACTOR UNLESS SPECIFIED ON THEPLANS TO BE ABANDONED.14. THE DEPARTMENT OF PUBLIC WORKS SHALL HAVE ACCESS TO THE WORK AT ALLTIMES DURING CONSTRUCTION AND SHALL BE FURNISHED WITH EVERYREASONABLE FACILITY (INCLUDING BUT NOT LIMITED TO LAYOUT UNDER THESUPERVISION OF A LAND SURVEYOR LICENSED IN THE STATE OF HAWAI'I) FORASCERTAINING THAT THE MATERIALS USED AND THE WORKMANSHIP ARE INACCORDANCE WITH THE REQUIREMENTS OF THE CHAPTER 9 SUBDIVISIONORDINANCE; ORDINANCE NO. 808, ARTICLE 7, GRADING, GRUBBING ANDSTOCKPILING; AND THE STANDARDS ESTABLISHED BY THE DEPARTMENT OFPUBLIC WORKS.15. UPON COMPLETION OF CONSTRUCTION, THE CONTRACTOR WILL PREPARE ANDCERTIFY AS-BUILT DRAWINGS THAT REPRESENT ALL CHANGES TO THE ORIGINALAPPROVED CONSTRUCTION PLANS. THE OWNER'S LICENSED DESIGN ENGINEERWILL CERTIFY THAT ALL OF THE CHANGES SHOWN ON THE AS-BUILT DRAWINGSHAVE BEEN APPROVED BY THE DESIGN ENGINEER AND MEET MINIMUMSTANDARDS IN ACCORDANCE WITH THE REQUIREMENTS OF THE ORIGINALAPPROVED CONSTRUCTION PLANS. THE DEPARTMENT OF PUBLIC WORKS MAYREQUIRE EVERY REASONABLE FACILITY TO VERIFY ANY IMPROVEMENT ON THEFINAL AS-BUILT PLANS PRIOR TO THE CERTIFICATION BY THE OWNER'SENGINEER, INCLUDING BUT NOT LIMITED TO HAVING A LAND SURVEYOR LICENSEDIN THE STATE OF HAWAI'I CONFIRM AND CERTIFY THE FINAL LOCATION ANDELEVATIONS.16. ALL TRAFFIC SIGNS AND PAVEMENT MARKINGS SHALL CONFORM TO THE MANUALON UNIFORM TRAFFIC CONTROL DEVICES FOR STREETS AND HIGHWAYS, CURRENTEDITION.17. THE CONTRACTOR SHALL MAKE PROPER ARRANGEMENTS FOR THE USE OFUTILITIES SUCH AS ELECTRICITY, WATER, ETC. AND SHALL BE RESPONSIBLE FORTHE PAYMENT OF ALL COSTS RESULTING FROM SUCH USAGE. THE CONTRACTORSHALL NOTIFY ALL AFFECTED UTILITY COMPANIES AND GOVERNMENT AGENCIESOF THEIR INTENT TO BEGIN CONSTRUCTION ON ANY INTERSECTION OR STREET ATLEAST TWO (2) WEEKS PRIOR TO THE START OF SUCH CONSTRUCTION.18. THE CONTRACTOR SHALL SCHEDULE ROAD AND BRIDGE WORK WITH THEDEPARTMENT OF PUBLIC WORKS, COUNTY OF KAUA'I AND NOTIFY THE PUBLIC NOSHORTER THAN ONE (1) MONTH PRIOR TO CONDUCTING THE WORK.19. THE CONTRACTOR SHALL CONTACT THE DEPARTMENT OF PUBLIC WORKS, DIVISIONOF ENGINEERING, CONSTRUCTION INSPECTION SECTION AT LEAST FIVE (5)WORKING DAYS BEFORE COMMENCING ANY WORK ON THE PROJECT. THECONTRACTOR SHALL HOLD A PRECONSTRUCTION MEETING WITH THECONSTRUCTION - DESIGN SECTION OF THE DEPARTMENT OF PUBLIC WORKSBEFORE COMMENCING ANY WORK.20. CONTRACTOR SHALL SUBMIT 'AS-BUILT" PLANS WITHIN 14 DAYS AFTER FINALINSPECTION.21. PRIOR TO THE START OF WORK, CONTRACTOR SHALL VISIT JOBSITE AND CONDUCTA PRE-CONSTRUCTION INSPECTION TO VERIFY QUANTITIES AND REPORT ANYDISCREPANCIES AND/OR DIFFERENCES IN DRAWINGS, IN RESPECT TO EXISTINGCONDITION, TO THE ENGINEER.22. CONTRACTOR SHALL RESOLVE ALL DISCREPANCIES AND QUESTIONS PRIOR TOSTART OF WORK. NO EXTRA PAYMENT SHALL BE ALLOWED ON ACCOUNT OF WORKMADE NECESSARY BY CONTRACTOR'S FAILURE TO VISIT THE JOBSITE AND/ORFAILURE TO RESOLVE DISCREPANCIES AND QUESTIONS.23. THE CONTRACTOR IS TO COMPLY WITH THE DIRECTIONS OF THE STATE OFHAWAI'I OCCUPATIONAL SAFETY AND HEALTH LAW (DOSH).1. ALL DAMAGED PAVEMENT SHALL BE RESTORED TO ITS ORIGINAL CONDITION INACCORDANCE WITH COUNTY OF KAUA'I, “HAWAI'I STANDARD SPECIFICATIONS FORROAD AND BRIDGE CONSTRUCTION, 2005” AND ITS AMENDMENTS AND THE“STANDARD DETAILS FOR PUBLIC WORKS CONSTRUCTION, SEPTEMBER 1984”, ASAMENDED BY THE DEPARTMENT FOR PUBLIC WORKS, CITY AND COUNTY OFHONOLULU, AND THE COUNTIES FOR KAUA'I, MAUI, AND HAWAI'I, WITH 2” MINIMUMHOT MIXED ASPHALT CONCRETE PAVEMENT (STATE DESIGN MIX V) AND 8”MINIMUM BASE COURSE UNLESS OTHERWISE NOTED IN THE PLANS.2. THE CONTRACTOR SHALL PROVIDE, INSTALL, MAINTAIN ALL NECESSARY SIGNS,LIGHTS, FLARES, BARRICADES, MARKERS, CONES AND OTHER PROTECTIVEFACILITIES AND SHALL TAKE ALL NECESSARY PRECAUTIONS FOR THEPROTECTION AND FOR THE CONVENIENCE AND SAFETY OF THE PUBLIC TRAFFIC.ALL SUCH PROTECTIVE FACILITIES AND PRECAUTIONS TO BE TAKEN SHALLCONFORM WITH RULES AND REGULATIONS GOVERNING THE USE OF TRAFFICCONTROL DEVICES AT WORK SITES ON OR ADJACENT TO PUBLIC STREETS ANDHIGHWAYS ADOPTED BY THE HIGHWAY SAFETY COORDINATOR AND U.S. FEDERALHIGHWAY ADMINISTRATION MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES FORSTREETS AND HIGHWAYS DATED 2009 AND ITS AMENDMENTS.3. A. A PERMIT SHALL BE OBTAINED BY THE CONTRACTOR FROM THEDEPARTMENT OF PUBLIC WORKS, COUNTY OF KAUA'I BEFORE WORK ON A PUBLIC STREET OR HIGHWAY MAY BEGIN. PERMIT FEES SHALL BE AT THECONTRACTOR'S EXPENSE.B. DRIVEWAYS SHALL BE KEPT OPEN UNLESS OWNERS OF THE ABUTTINGLOTS USING THESE RIGHT-OF-WAY ARE OTHERWISE PROVIDED FOR SATISFACTORILY.C. ALL WORK INCLUDING REPAIR OF DAMAGED PAVEMENT AND SHOULDERSSHALL BE INSPECTED AND APPROVED BY THE DEPARTMENT OF PUBLIC WORKS. ALL UNAPPROVED WORK SHALL BE CONSIDERED UNACCEPTABLE AND SHALL BE RE-WORKED AND CORRECTED AS DIRECTED BY THE DEPARTMENT OF PUBLIC WORKS, AT THE CONTRACTOR'S EXPENSE.D. NO MATERIAL AND/OR EQUIPMENT SHALL BE STOCKPILED OR OTHERWISESTORED WITHIN COUNTY RIGHT-OF-WAY EXCEPT AT LOCATIONSDESIGNATED IN WRITING AND APPROVED BY THE COUNTY ENGINEER.H. TEMPORARY DUST CONTROL MEASURES FOR GRADING
1. THE GRADED OR PROJECT SITE THAT IS CLEARED OF VEGETATION SHALL BE
KEPT DAMP WITH WATER CONTINUOUSLY FOR SEVEN (7) DAYS A WEEK. AT THE
END OF EACH DAY, THE SITE SHALL BE SUFFICIENTLY DAMPENED WITH WATER
ON A CONTINUAL BASIS SO THAT THE SITE WILL REMAIN MOISTENED DURING
THE NIGHT.
2. THE CONTRACTOR SHALL CONDUCT HIS OPERATIONS SO THAT EXCAVATION,

SCHEDULES AND/OR WRITTEN MEASURES WHICH PROVIDES FOR DUST CONTROL.
THE DUST CONTROL MEASURES SHALL CONTAIN POSITIVE STATEMENTS WHICH
REQUIRE ACTIONS OR WORK THAT PREVENT DUST PROBLEMS. NO PERMITS WILL
BE ISSUED UNLESS THE COUNTY IS ASSURED THAT DUST PROBLEMS WILL BE
MINIMIZED.
TEMPORARY EROSION CONTROL MEASURES FOR GRADING
1. TEMPORARY VEGETATIVE COVER SHALL BE PLANTED WITHIN A PERIOD OF 30
CALENDAR DAYS AFTER THE SITE HAS BEEN GRADED OR BARED OF
VEGETATION OR IF THE SITE WILL BE SUSPENDED FOR MORE THAN 30
CALENDAR DAYS.
2. TEMPORARY VEGETATIVE COVER SHALL CONSIST OF 40 LBS. COMMON RYE GRASS
SEED PER ACRE, 400 LBS. PER ACRE 10-10-10 OR EQUIVALENT FERTILIZER
WORKED INTO THE SEED BED BEFORE PLANTING. TEMPORARY SPRINKLER
SYSTEM IS TO BE INSTALLED CONCURRENTLY WITH ALL PLANTINGS AND
MAINTENANCE OF GRASS SHALL CONFORM TO THE “HAWAI'I STANDARD
SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION, 2005” AND ITS
AMENDMENTS.
PERMANENT EROSION CONTROL MEASURES FOR GRADING
1. THE CONTRACTOR SHALL GRASS THE ENTIRE PROJECT SITE, EXCEPT PAVED
AREAS WITH BERMUDA GRASS SPRIGS. THE GRASS SHALL BE PLANTED,
FERTILIZED, AND MAINTAINED IN ACCORDANCE WITH THE “HAWAI'I STANDARD
SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION, 2005” AND ITS
AMENDMENTS.
2. THE CONTRACTOR SHALL GRASS ALL EXPOSED AREAS THAT HAVE BEEN
CONSTRUCTED TO FINAL GRADES WITHIN A PERIOD OF 30 CALENDAR DAYS.
3. IN LIEU OF GRASS SPRIGS (NOTE 1), THE CONTRACTOR MAY USE HYDROMULCH
WITH SEEDINGS AND IRRIGATION SPRINKLER SYSTEM.
GRADING PHASES
1. WHEN GRADING WORK IS DONE IN PHASES, THE ENGINEER MUST ACCEPT THE
COMPLETED PHASE PRIOR TO START OF WORK ON THE NEXT PHASE. EVEN
AFTER A COMPLETED PHASE HAS BEEN ACCEPTED, THE GRASSING OR OTHER
MEANS OF STABILIZATION MUST BE MAINTAINED UNTIL PROJECT COMPLETION.
1. IN ACCORDANCE WITH CHAPTER 11-60.1, AIR POLLUTION CONTROL, TITLE 11,
HAWAI'I ADMINISTRATIVE RULES, THE CONTRACTOR SHALL BE RESPONSIBLE
FOR ENSURING THAT EFFECTIVE CONTROL MEASURES ARE PROVIDED TO
MINIMIZE OR PREVENT ANY VISIBLE DUST EMISSION CAUSED BY THE
CONSTRUCTION WORK FROM IMPACTING THE SURROUNDING AREAS INCLUDING
THE OFF-SITE ROADWAYS USED TO ENTER/EXIT THE PROJECT. THESE
MEASURES INCLUDE BUT ARE NOT LIMITED TO THE USE OF WATER WAGONS,
SPRINKLER SYSTEM, DUST FENCES, ETC.
2. IN ACCORDANCE WITH CHAPTER 11-55, WATER POLLUTION CONTROL AND
CHAPTER 11-54, WATER QUALITY STANDARDS, TITLE 11, HAWAI'I
ADMINISTRATIVE RULES, THE CONTRACTOR SHALL BE RESPONSIBLE FOR
ENSURING THAT THE BEST MANAGEMENT PRACTICES (BMP) TO MINIMIZE OR
PREVENT THE DISCHARGE OF SEDIMENTS, DEBRIS AND OTHER WATER
POLLUTANTS INTO STATE WATERS IS PROVIDED AT ALL TIMES.
3. IN ACCORDANCE WITH CHAPTER 11-58, SOLID WASTE MANAGEMENT CONTROL,
TITLE 11, HAWAI'I ADMINISTRATIVE RULES, THE CONTRACTOR SHALL BE
RESPONSIBLE FOR ENSURING THAT GRUB MATERIAL, DEMOLITION WASTE AND
CONSTRUCTION WASTE GENERATED BY THE PROJECT ARE DISPOSED OF IN A
MANNER OR AT A SITE APPROVED BY THE STATE DEPARTMENT OF HEALTH.
DISPOSAL OF ANY OF THESE WASTES BY BURNING IS PROHIBITED.
4. THE CONTRACTOR SHALL BE RESPONSIBLE FOR OBTAINING AND PAYING FOR ALL
APPLICABLE PERMITS FROM THE DEPARTMENT OF HEALTH INCLUDING BUT NOT
LIMITED TO NATIONAL POLLUTION DISCHARGE ELIMINATION SYSTEM (NPDES),
NOTICE OF INTENT AND GENERAL PERMIT FOR STORM WATER, HYDROSTATIC
TEST AND DEWATERING DISCHARGES PRIOR TO COMMENCING CONSTRUCTION.
NPDES PERMIT SHALL BE REQUIRED PRIOR TO GRADING OR GRUBBING WORK
OVER AN AREA OF ONE ACRE OR MORE. AN NPDES PERMIT IS NOT REQUIRED
FOR THIS WORK.
5. AFTER EACH RAINFALL EVENT, THE CONTRACTOR SHALL REMOVE ALL SILT AND
DEBRIS RESULTING FROM THE WORK AND DEPOSITED IN DRAINAGE FACILITIES,
ROADWAYS AND OTHER AREAS. THE COST INCURRED FOR ANY NECESSARY
REMEDIAL ACTION BY THE COUNTY ENGINEER SHALL BE PAYABLE BY THE
CONTRACTOR.
6. BEST MANAGEMENT PRACTICES (BMP'S) SHALL BE EMPLOYED AT ALL TIMES TO
THE MAXIMUM EXTENT PRACTICABLE TO PREVENT DAMAGE BY SEDIMENTATION,
EROSION OR DUST TO STREAMS, WATER COURSES, NATURAL AREAS AND THE
PROPERTY OF OTHERS.
7. THE CONTRACTOR SHALL OBTAIN AND COMPLY WITH NATIONAL POLLUTION
DISCHARGE ELIMINATION SYSTEM (NPDES) PERMIT REQUIREMENTS FOR ALL
PROJECTS WHICH WILL DISTURB ONE (1) ACRE OR MORE OF LAND. THE
CONTRACTOR SHALL NOT START CONSTRUCTION UNTIL NOTICE OF GENERAL
PERMIT COVERAGE (NGPC) IS RECEIVED FROM THE DEPARTMENT OF HEALTH,
STATE OF HAWAI'I AND HAS SATISFIED ANY OTHER PERMITTING REQUIREMENTS
OF THE NPDES PERMIT PROGRAM. AN NPDES PERMIT IS NOT REQUIRED FOR
THIS WORK.
8. IN ACCORDANCE WITH CHAPTER 11-46, COMMUNITY NOISE CONTROL, TITLE 11,
HAWAI'I ADMINISTRATIVE RULES, THE CONTRACTOR SHALL BE RESPONSIBLE
FOR PROVIDING EFFECTIVE CONTROL MEASURES TO MINIMIZE OR PREVENT
CONSTRUCTION RELATED NOISE FROM IMPACTING THE RESIDENTS IN THE
IMMEDIATE AREA. IF REQUIRED, NOISE REDUCTION MEASURES SHALL BE
IMPLEMENTED BY THE CONTRACTOR DURING THE CONSTRUCTION WORK.
9. THE PROPERTY MAY HARBOR RODENTS WHICH WILL BE DISPERSED TO THE
SURROUNDING AREAS WHEN THE SITE IS CLEARED. IN ACCORDANCE WITH
CHAPTER 11-26, VECTOR CONTROL, TITLE 11, HAWAI'I ADMINISTRATIVE RULES,
THE CONTRACTOR SHALL ASCERTAIN THE PRESENCE OR ABSENCE OF RODENTS
ON THE PROPERTY. SHOULD THE PRESENCE OF RODENTS BE DETERMINED, THE
CONTRACTOR SHALL ERADICATE THE RODENTS PRIOR TO CLEARING THE SITE.
10. A COPY OF THE PLANS, CONSTRUCTION SCHEDULE AND/OR WRITTEN MEASURES
THAT ARE REQUIRED TO BE SUBMITTED BY THE CONTRACTOR (DUST CONTROL
MEASURES/PLANS) SHOULD ALSO BE SENT TO THE DEPARTMENT OF HEALTH
FOR MONITORING PURPOSES.
11. CONTRACTOR SHALL AVOID CONTACT WITH ANY PROTECTED SPECIES OBSERVED
IN CLOSE PROXIMITY TO THE REPAIR ACTIVITIES. CONTRACTOR SHALL TAKE
IMMEDIATE ACTION TO AVOID DISTURBANCE TO THE REGULAR BEHAVIOR AND
ACTIVITIES OF THE ANIMAL. ANY INTERACTION BETWEEN A PROTECTED SPECIES
AND THE REPAIR ACTIVITIES SHOULD BE REPORTED TO THE ENGINEER
IMMEDIATELY.
1. PAVEMENT RESURFACING WORK SHALL INCLUDE 2-INCH THICKNESS OF
EXISTING A.C. TO BE COLD PLANED AND CONSTRUCTION OF A MINIMUM OF
2-INCH OF NEW A.C. (STATE PMA MIX IV) LAYER UNLESS OTHERWISE NOTED IN
THE PLANS. THE MILLED AREA SHALL BE PREPARED BY REMOVING ALL DIRT
AND DEBRIS, OILS, AND ANY POTENTIAL BONDING INHIBITORS. A TACK COAT
AND A LAYER OF TENSAR RAPID REPAIR PG100 OR APPROVED EQUAL SHALL BE
APPLIED BEFORE PLACING THE ASPHALT OVERLAY.
2. ALL EXISTING PAVEMENT STRIPING DISTURBED BY THIS PROJECT SHALL BE
RESTORED. THE STRIPING MATERIALS SHALL BE THERMOPLASTIC TAPE OR
THERMOPLASTIC EXTRUSION. PAINTING IS NOT ACCEPTABLE.
3. THE LIMITS OF ROAD RESTORATION WORK MAY BE REVISED BY THE
ENGINEERING DIVISION OF THE COUNTY DEPARTMENT OF PUBLIC WORKS

GENERAL:
A. WORKMANSHIP AND MATERIALS SHALL CONFORM TO THE AASHTO LRFD BRIDGE DESIGN SPECIFICATION, 9TH EDITION
(INCLUDING MOST RECENT INTERIMS), AND THE HAWAII STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE
CONSTRUCTION (2005 EDITION), AND ALL APPLICABLE SPECIAL PROVISIONS BY THE STATE OF HAWAII DEPARTMENT
OF TRANSPORTATION.
B. THE CONTRACTOR SHALL COMPARE ALL THE CONTRACT DOCUMENTS WITH EACH OTHER AND REPORT IN WRITING TO
THE ENGINEER ALL INCONSISTENCIES AND OMISSIONS.
C. THE CONTRACTOR SHALL TAKE FIELD MEASUREMENTS AND VERIFY FIELD CONDITIONS AND SHALL COMPARE SUCH
FIELD MEASUREMENTS AND CONDITIONS WITH THE DRAWINGS BEFORE COMMENCING WORK. REPORT IN WRITING TO
THE ENGINEER ALL INCONSISTENCIES AND OMISSIONS.

a. PREPACKAGED CEMENTITIOUS PATCHING MORTAR SHALL BE POLYMER MODIFIED, HAVE HIGH ABRASION
RESISTANCE, AND SHALL BE SUITABLE FOR OVERHEAD AND VERTICAL SURFACES.
b. THE MINIMUM BOND STRENGTH PROVIDED BY THE PATCHING MORTAR SHALL BE 2,000 PSI AFTER 28 DAYS
(ASTM C-882).
c. THE MINIMUM COMPRESSIVE STRENGTH PROVIDED BY THE PATCHING MORTAR SHALL BE 6,000 PSI AFTER
28 DAYS (ASTM C-882).

ALKALIS, SALTS, ORGANIC MATERIALS OR OTHER SUBSTANCES THAT ARE DELETERIOUS TO CONCRETE OR
STEEL REINFORCEMENT.
3. PRECAST WALKWAY SLAB - 5,000 PSI COMPRESSIVE STRENGTH AT 28 DAYS
a. A CORROSION INHIBITING ADMIXTURE SHALL BE INCLUDED IN THE CONCRETE MIX. THE ADMIXTURE SHALL
BE RHEOCRETE CNI CORROSION INHIBITOR FROM BASF, DCI S CORROSION INHIBITOR FROM GRACE
CONSTRUCTION PRODUCTS OR AN APPROVED EQUAL. ADDITION RATE OF CORROSION INHIBITING
ADMIXTURE SHALL BE AS RECOMMENDED BY THE MANUFACTURER.
B. ALL NON-STAINLESS STEEL OR ALUMINUM INSERTS, ANCHOR BOLTS, PLATES, AND OTHER ITEMS TO BE CAST IN THE
CONCRETE SHALL BE HOT-DIP GALVANIZED ACCORDING TO ASTM A153 OR ASTM 123 UNLESS OTHERWISE NOTED.
C. REINFORCING BARS, ANCHOR BOLTS, INSERTS, AND OTHER ITEMS TO BE CAST IN THE CONCRETE SHALL BE SECURED
IN POSITION PRIOR TO PLACEMENT OF CONCRETE USING EPOXY IN A MANNER NOT TO PROHIBIT THE REQUIRED
CONCRETE ADHESION.
D. EPOXY FOR DOWELS SHALL BE HILTI HIT-RE 500-V3, OR APPROVED EQUAL.
E. EPOXY FOR CRACK REPAIRS SHALL BE SIKADUR-52 INJECTION, OR APPROVED EQUAL. APPLY IN ACCORDANCE WITH
MANUFACTURERS RECOMMENDATIONS.
EXISTING CONCRETE:
A. CONTRACTOR SHALL NOT DAMAGE, CUT OR DRILL THROUGH EXISTING REINFORCING. IF REINFORCING IS DAMAGED,
THE CONTRACTOR SHALL INFORM THE CONTRACTING OFFICER IMMEDIATELY AND SHALL BE RESPONSIBLE FOR
REPAIRING THE DAMAGE AT CONTRACTOR'S SOLE EXPENSE AND TO THE SATISFACTION OF THE CONTRACTING
OFFICER.
COATING OF SPALL REPAIRS:
A. ALL SPALL REPAIRS SHALL BE COATED AFTER THE PATCHING MATERIAL HAS CURED FOR A MINIMUM OF 14 DAYS.
B. THE LIMITS OF THE COATING APPLICATION SHALL INCLUDE THE FOLLOWING:
1. 100% COVERAGE OF THE SPALL REPAIR AREA
2. 6” BAND OF EXISTING CONCRETE SURFACE AROUND THE PERIMETER OF THE SPALL REPAIR AREA
C. THE COATING SHALL CONSIST OF A HIGH-SOLIDS, HIGH-BUILD AMINE EPOXY COATING THAT CAN WITHSTAND
ABRASION. SIKAGARD 62, OR APPROVED EQUAL. THE COATING SHALL BE APPLIED UNTIL A TOTAL WET FILM
THICKNESS OF 14 MILS IS ACHIEVED. APPLY IN TWO COATS OF 7 MILS EACH.
D. REMOVE ALL FORM RELEASE AGENTS, CURING COMPOUNDS, SALTS, EFFLORESCENCE, LAITANCE, AND OTHER FOREIGN
MATTER BY SANDBLASTING, MECHANICAL SCARIFICATION, OR SUITABLE CHEMICAL MEANS. REFER TO ASTM D4260.
RINSE THOROUGHLY AND ALLOW TO COMPLETELY DRY BEFORE COATING.

3. THE EXPOSED CONCRETE SURFACE SHALL BE ROUGHENED TO A
1 8" AMPLITUDE AND SHALL BE CLEANED AND FREE OF LAITANCE, DUST, GREASE, AND ALL
OTHER BOND-INHIBITING MATERIALS FROM THE REBAR AND CONCRETE SURFACES WITHIN THE REPAIR AREA. CLEAN EXPOSED REINFORCING BY HAND OR WITH
POWER TOOLS SO IT IS FREE OF LOOSE RUST AND OTHER LAITANCE PER SSPC-SP2 OR SSPC-SP3.
4. INSPECT THE CONDITION OF THE NEWLY EXPOSED REBARS. WHEN CORROSION HAS CAUSED >25% SECTION LOSS OF THE REBARS, THE CONTRACTOR SHALL
SPLICE A NEW SECTION OF REBAR INTO PLACE, AS SHOWN IN DETAIL 2/S8.2. COAT ALL EXPOSED REINFORCING WITH 2 COATS OF EPOXY BONDING AGENT.
EPOXY BONDING AGENT SHALL BE SIKA ARMATEC 110, MASTER BUILDERS EMACO P24, EUCLID CORR-BOND OR APPROVED EQUAL. EACH COAT SHALL BE 20 MILS
THICK.
5. FORM AND PUMP ALL OVERHEAD REPAIRS GREATER THAN 4 SQUARE FEET. ALL OTHER REPAIRS MAY BE TROWEL APPLIED.
6.FOR TROWEL APPLIED REPAIRS ONLY: COAT CONCRETE SURFACE WITH 20 MIL THICK EPOXY BONDING AGENT. SURFACE SHALL BE SATURATED SURFACE DRY
(SSD) AND CLEANED FREE OF LAITANCE, DUST AND OTHER BOND INHIBITING MATERIAL.
7. CONCRETE REPAIR MATERIALS SHALL BE A POLYMER-MODIFIED CEMENT MORTAR. POLYMER MODIFIED CEMENT MORTAR FOR TROWEL APPLIED REPAIRS SHALL BE
SIKA SIKATOP 123 PLUS, MASTER BUILDERS EMACO R320 CI, EUCLID VERTICOAT OR APPROVED EQUAL. POLYMER MODIFIED CEMENT MORTAR FOR FORMED AND
PUMPED REPAIRS SHALL BE SIKA SIKATOP 111 PLUS, OR APPROVED EQUAL.
8. LAYERS OF TROWEL APPLIED POLYMER-MODIFIED CEMENT MORTAR SHALL BE
